Our client is seeking an experienced Behaviour Support Practitioner to join their multi-disciplinary West Melbourne service.

Key responsibilities
Conduct comprehensive functional assessments and design, implement and monitor behaviour support interventions for people with disability exhibiting behaviours of concern
Deliver timely, efficient and effective behaviour support services aligned to participant and family/carer goals
Plan, deliver and evaluate support strategies in accordance with all relevant legislative and ethical requirements
Collaborate closely with participants, families, carers and the wider support network to ensure continuity of care
Maintain awareness of KPIs: Whilst exceptional healthcare remains the priority, you will work consistently to achieve agreed targets

Essential requirements
Current registration with the Quality and Safeguards Commission as a Behaviour Support Practitioner (Core, Proficient, Advanced and/or Specialist)
Demonstrated experience delivering behaviour support to children, adolescents and/or adults with complex support needs (e.g. mental health diagnoses, health needs, complex trauma)
Current driver’s licence and access to a registered vehicle
